Get Healthy in a Few Easy Steps:

1. Please contact our office at (714) 540-0555 to schedule your New Patient application. We do require full payment of the first appointment upon scheduling to lock in your appointment date. If appointments are needing to be rescheduled we require a 24 hour notice and original payment will be applied to your first visit. 

2. Once your appointment is scheduled you will be emailed a confirmation email that will include our office details and a link to fill out your intake information online. Make sure to click "Update Forms" at the bottom to save all your work. No need to print it out it will reflect on our end. This is required to be completed 24 hours before your scheduled appointment time. 

3. If you have any lab work, X-Rays images/ reports, MRI reports, CT-Scan reports , please either email them to reception@nmccenters.com before your appointment or come to your appointment 15 minutes early to have them scanned in and uploaded before your visit with the Doctor. 

4. Once you arrive at your scheduled appointment time, we have a few forms to sign and you will be on your way to a healthier you!
